Viral Hepatitis MOC Database Resource Entry

Untitled document

Guidelines for submission

Thank you for submitting a resource to be included on the Viral Hepatitis Model of Care database. Please ensure the document does not already exist in the Viral Hepatitis Models of Care database before submitting a new resource. Please complete all fields of the following submission form. An ID number will be allocated automatically.

Your submission will be considered and may be edited prior to entry. ASHM cannot guarantee acceptance of a submission onto the database. You will be notified via email of the outcome of your submission.

Notes:

  • Origin represents the continent or country of origin of the resource.
  • If you are submitting a new resource that is not already available elsewhere online, please tick the "I will supply resource document to ASHM" box. It will then be made accessible through the website. You will be furnished with an email address to send your document to upon submission.
  • Doctype and Size fields can hold multiple respective values. For example, if a resource is available in both PDF and DOC format, then you would comma-separate the document types and sizes - i.e. 'DOC, PDF' and '1.2Mb, 780kb'.
  • Publisher's description is usually available from the originating website.
  • URLs:
    • The http:// component of a URL is already implicit and does not need to be entered.
    • Only one URL can be entered for each resource.
    • In most cases, the URL should represent the link to the page from which the document is linked and NOT the document itself. This reduces problems when websites are re-organized.
  • Please include keywords that clearly define the content and purpose of the resource. Key words should include the relevant descriptor: HBV, HCV and/or Viral hepatitis. Include those words which may not already exist in the title or description for the resource. Add as many as you wish, (e.g. clinical management education guidelines testing). Do not separate terms by commas, semicolons etc.
  • Please try to avoid the use of odd punctuation, characters and symbols as these may corrupt the data. This includes "double quotation" marks.
