New Membership Application
Upon submitting your application, you will recieve notice via email that your application is pending. Following approval form the ASHM Board, you will be notified that your application has been accepted and will receive a formal welcome pack in the mail to your prefered postal address. Your first year of recorded Membership will run from the date your application is accepted until 31 December of that year. Subsequent Membership years run from 1 January to 31 December. Full Membership benefits will not be provided until the Membership application has been approved by the ASHM Board at their monthly meeting, and full payment has been received.

If you prefer not to provide information electronically you can also apply using the standard ASHM Membership application form. Please download the form below, print and complete all sections, and return to ASHM by fax: 02 921 2 2382 or post: LMB 5057, Darlinghurst NSW 1300. Following receipt of the Membership application and approval by the ASHM Board you will be sent a welcome pack and receipt.
As part of your Membership, you will receive relevant Email Alerts according to your nominated areas of interest. Please ensure you indicate your professional group and areas that you are especially interested in hearing about when you return the application form containing the required signatures.
