Registration & Payment
• Each delegate must register online
• No paper registration form will be available but if you are having problems with the online system please advise the conference secretariat and an alternative system will be arranged.
• Should you wish to register for both the ASHM and Sexual Health Conferences please refer to the Joint Conference rates information on the website
• The registration cut-off date is Tuesday 2 September 2008. Registrations received after this date will be processed on site and will incur the A$55 late registration fee.
• Payment can be made by cheque, bank draft in Australian dollars (A$) or credit card (Bankcard, Visa or MasterCard). Cheques must be made payable to "ASHM"
• Payment must accompany all registration applications as reservations will not be confirmed unless payment is received
• Invoices are provided with your confirmation letter which are sent out periodically

Earlybird Rate
To qualify for the Early Bird rate your registration and accompanying payment must be received online by 5.00pm on or before Thursday 17 July 2008. The Early Bird rate will not be given if payment is not received by that date and the standard rate will apply.
Member Rate
To receive the Member Rate for the ASHM Conference you must be a member of ASHM, or an employee or member of a collaborating centre which includes ACH2, ARCSHS, NCHECR and NCHSR.
Earlybird or Standard Rate Including ASHM Membership
All ASHM Full Members and Associate Members can register for the ASHM Conference at the reduced
member's rate. Non members can take advantage of this saving now by registering under the ‘earlybird or standard rate (depending on the date you are registering) including the Membership fee'. If you take up this option you will be automatically charged for the registration and membership fee rate. A membership form will be emailed to you, so you can fill out your details; however you have already paid to become an ASHM member. Registering under this rate means you are signing as a 2008 member (contingent to your application form being accepted)
Student and Research Assistant Rate
To be eligible for the reduced student and research assistant rate all the following conditions need to be met:
• Students must be in full-time study (please attach a photocopy of your Student ID with your registration form)
• Registration must not be otherwise funded
• Supervisors must sign the appropriate section on the registration Form
PLWHA / People with Hepatitis C Rate, ASHM ONLY
To be eligible for the reduced PLWHA / People with hepatitis C rate one of the following conditions needs to be met:
• Applicants must be on a health card (please send a photocopy) or
• Applicants must be endorsed by NGO (tick the appropriate section on the online registration form)
Middle/Low Income Countries Rate
To encourage broad participation in the conference, we are offering registration fees for delegates from middle/
low income countries. The following 56 countries are considered high income economies by the World Bank:
Andorra, Antigua and Barbuda, Aruba, Australia, Austria, Bahamas (The), Bahrain, Belgium, Bermuda, Brunei
Darussalam, Canada, Cayman Islands, Channel Islands, Cyprus, Denmark, Faeroe Islands, Finland, France, French Polynesia, Germany, Greece, Greenland, Guam, Hong Kong (China), Iceland, Ireland, Isle of Man, Israel, Italy, Japan, Korea, Rep., Kuwait, Liechtenstein, Luxembourg, Macao (China), Malta, Monaco, Netherlands, Netherlands Antilles, New Caledonia, New Zealand, Norway, Portugal, Puerto Rico, Qatar, San Marino, Saudi Arabia, Singapore, Slovenia, Spain, Sweden, Switzerland, United Arab Emirates, United Kingdom, United States and Virgin Islands (U.S.).
Joint Conferences Rates/Procedures (rates are shown in the table attached)
ASHM & Sexual Health - To register for these two conferences which entitles the participant to the conference program from Monday 15 - Saturday 20 September 2008 please complete one form only available on the conference website.
Late Registration Fees
A$55.00 will be added to all registration fees (excluding day registrations) when registering after Tuesday 2 September 2008 or onsite. Note: If registering late, we cannot guarantee the availability of function tickets.
Acknowledgement
Confirmation of registration, hotel bookings and payment will be acknowledged in writing until Tuesday 2 September 2008. After this date, verbal acknowledgement can be obtained by calling the secretariat on +61 2 8204 0770. If you do not receive your acknowledgement please contact the conference secretariat on the above number. For registrations paid by organisations, please check with your accounts department to confirm that the registration form and payment have been forwarded to the conference secretariat.
Cancellations and Amendments
Registration and/or accommodation cancellations and amendments must be notified in writing to the secretariat. Cancellations received before Monday 11 August 2008 will receive a refund less A$150.00 administration fee. Refunds (less administration fee) for cancellations received in writing after Monday 11 August 2008 will be made only in exceptional circumstances. As an alternative to cancellation, your registration may be transferred to another person without incurring any penalty. The conference secretariat must be advised of the transfer in writing. Any outstanding refunds will be paid after the conclusion of the conference. A fee may be charged to your credit card by the hotel, according to their cancellation conditions when cancelling accommodation bookings after Monday 11 August 2008
